In accordance with 42 C.F.R. § 416.50, this disclosure is provided prior to your scheduled surgical procedure. A current list of all physician owners is maintained by the facility and is available for review upon request.

This disclosure is intended to assist you in making an informed decision regarding your care. You have the right to select the healthcare provider of your choice and are not obligated to receive services at this facility.

The physicians and other specialized medical service providers, including anesthesia, radiology or pathology, which use this facility, are independent contractors and are not employees or agents of the facility. Since they are independent contractors, the facility is not responsible or liable for their acts or omissions.
